Thrombocytopenia is the abnormally low levels of thrombocytes (platelets) in the blood. A normal platelet count ranges from 150,000 to 450,000 platelets per microliter of blood. Having more than 450,000 platelets is a condition called thrombocytosis; having less than 150,000 is known as thrombocytopenia.

SIGNS AND SYMPTOMS OF THROMBOCYTOPENIA

  • Delay in the clotting of blood after bleeding
  • Gum bleeding
  • Epistaxis
  • Blood in vomit and stools
  • Blood in urine
  • Purpura
  • Headache
  • Muscle pain

CAUSES OF THROMBOCYTOPENIA

  • Leukaemia
  • Radiation therapy
  • Chemotherapy
  • Autoimmune diseases
  • Aplastic anaemia
  • Splenic enlargement
  • Liver cirrhosis
  • Certain medications
  • Dengue fever

PATHOPHYSIOLOGY OF THROMBOCYTOPENIA

Due to the above-mentioned causes, platelet count decreases lead to delayed blood clotting. There is a chance of bleeding through the mucous membrane and skin.

DIAGNOSIS OF THROMBOCYTOPENIA

  • Platelet count
  • Complete blood count
  • Bleeding time
  • Clotting time
  • Bone marrow biopsy
  • USG scan
  • CT scan

TREATMENTS OF THROMBOCYTOPENIA

  • Blood transfusion
  • Oral corticosteroids
  • Fluid supplementation
  • Treatment for underlying pathology

PROGNOSIS OF THROMBOCYTOPENIA

Generally, the symptoms are manageable. In the case of severe thrombocytopenia need for a special medical emergency.

  • Dangerous internal bleeding

THROMBOCYTOPENIA AND AYURVEDA

  • In Ayurveda,thrombocytopenia management is based on ‘rakta pitta chikilsa’.

NIDANA- AYURVEDIC CAUSES OF THROMBOCYTOPENIA

  • Excess consumption of acrid, salt, hot,  ununctuous food
  • Excess vitiation of pitta dosha
  • Vitiation of rakta dhatu

PURVAROOPAM- AYURVEDIC PREMONITORY SYMPTOMS OF THROMBOCYTOPENIA

  • Taste of blood in the mouth
  • Bleeding from body orifices

SAMPRAPTI – AYURVEDIC PATHOGENESIS OF THROMBOCYTOPENIA

Due to the causative factors doshas become vitiated and spreads to the entire body. Aggravated doshas vitiate the rakta dhatu. If it is associated with the vitiation of kapha, leads to bleeding from the upper orifices, whereas if it is associated with vitiated vata, leads to bleeding from the lower orifices.

  • Sandhi Shoola – joint pain
  • Shirah Shoola – headache

AYURVEDIC PROGNOSIS OF THROMBOCYTOPENIA

If it is associated with the vitiation of kapha, leads to bleeding from the upper orifices, whereas if it is associated with vitiated vata, leads to bleeding from the lower orifices. The first condition is curable and later is difficult to cure. If both kapha and vata are involved in the pathogenesis, then the complications arise and such conditions are incurable.

  • Avoid food items that produce dosha imbalance like acrid and dry food, hard to digest. Because dosha imbalance aggravates the symptoms.
  • Consume easily digestible food items

YOGA FOR THROMBOCYTOPENIA

  • Nadi Shuddi Pranayam   

The patient needs to be seated in a meditative posture with the head and spine erect, with the body relaxed. The patient has to close his one nostril (e.g. left nostril if using the right hand and vice versa) with the thumb and exhale completely through the other nostril. Again, he will have to breathe in deeply through the other nostril while the opposite nostril is still closed with the thumb.
