Small warts that occur inside and around the anus are known as anal warts. The condition is known as Condyloma acuminata. Anal warts are a type of genital warts. They may occur in a single spot or may spread to different parts of the genitals or anus over time.

Signs and Symptoms

Warts usually start as small bumps of size not larger than the head of a pin. They start as very minute growths with later on grows to a cauliflower-like appearance when they are clustered together. No pain occurs in anal warts. Rarely, symptoms like itching, bleeding, or discharge from the anus is seen. There may be a feeling of a lump in the anal region.

Causes

Anal warts, like other genital warts, are caused by the Human papillomavirus (HPV) which is a sexually transmitted one. HPV virus spread through direct contact with the mouth, anus, penis, or vagina. Genital warts commonly spread through anal or vaginal sex.

Sex at an early age, multiple sex partners, anal sex, etc. are all risk factors for the development of anal warts.

Pathophysiology

In the case of genital warts and anal warts, the HV virus invades cells of the basal layer of the epidermis by penetrating the skin and mucosal aberrations in the genital area. A latent period of 3 weeks to 9 months is taken.

Diagnosis

Diagnosis of anal warts is done by visual examination. For diagnosis of anal warts inside anal canal examination using anoscope is used. In females, diagnosis is confirmed by Pap smear.

Treatments

The treatment includes,

Topical applications of creams like imiquimod (Aldara, Zyclara), podofilox (Condylox), podophyllin (Podocon), etc. are used. This is successful if the warts are very small and limited to the outer area of the anus.
The techniques like Cryotherapy using liquid nitrogen, Electrotherapy, and laser techniques are also used in the treatment of anal warts.
In some cases, surgery is done.

Prognosis

Anal warts are not a life-threatening condition and only in rare cases, it may turn to cancer. But the HPV virus can remain in the body for a longer time and so the chance of recurrence is more.

Complications

Anal warts can sometimes progress to malignancy and spread through sexual contact.

Charmakeela is a condition in Ayurveda where a keel- a nail-like growth occurs on the skin. It is seen in the Charma of Guhya Pradesh- (skin of genital area and anus).

Nidana- Causes

It is described under Kshudraroga by Acharyas and no specific nidana is said by Acharyas.

Samprapthi Maturation

Vitiated vyanavayu aggravates and it gets associated with Kapha dosha, give rise to nail shaped immovable sprouts on the skin surface called Charmakeela or charmaarshas.

Lakshana Signs and Symptoms

The symptoms include,

Pricking pain
Growth has a knotty shape
Colour is usually similar to the colour of the surrounding area.
Sometimes they are dry, dark red-coloured, smooth with profound hardness due to pitta and raktha.

Prognosis

It can be cured by Para surgical procedures like Kshara karma or Agni karma.

Chikitsa- Treatment

The treatment said in classics include,

Agni karma
Kshara karma
Chedana(surgical excision)

The surgical excision of the wart followed by Agni karma or Kshara karma is usually beneficial.

Home Remedies

Tea tree oil has antifungal properties and so this can be applied to warts by mixing few drops of tea tree oil with coconut oil
Green tea is found to be effective against warts. Green tea extracts are available.
Applying extract of garlic on warts helps to clear them.
Apple cider vinegar is also beneficial for external applications.
Folate and multivitamin supplements including vitamin B12

Diet

The beneficial diet includes,

Vegetables like cabbage, broccoli, cauliflower, etc. have Indole-3-carbinol which is beneficial against anal warts.
Foods with antioxidant properties like dark green leafy vegetables, tomatoes, kale, whole grains, almonds, etc.

Also avoid foods like pasta, red meat, bread, dairy products, processed foods with trans fats.
