A varicose ulcer is formed as a complication of varicose vein. A vein is said to be varicose when it is dilated, lengthened and tortuous. A varicose vein is mainly seen in the leg but spermatic, oesophagal and haemorrhoidal veins may also be affected.

SIGNS AND SYMPTOMS OF VARICOSE ULCER

  • Leg ulcer
  • Skin rashes
  • Redness
  • Pus in the wound
  • Aching pain in the calf region
  • Ankle swelling
  • Night cramps
  • Itching sensation over the skin
  • Dragging pain in the leg
  • Pain may relive at night or elevation of the leg

CAUSES OF VARICOSE ULCER

  • Congenital absence of valves
  • The congenital paucity of valves
  • Venous dilatation
  • Wasting of muscles
  • Any obstruction which hampers venous return- pregnancy, tumours
  • Deep vein thrombosis
  • Certain medications 

PATHOPHYSIOLOGY OF VARICOSE ULCER

Due to the above-mentioned causative factors pooling of blood in the superficial veins and rise in intraluminal pressure occurs. Excessive dilatation occurs due to loss of elasticity, sacculation and elongation. When an elongated structure is placed between shorter lengths it becomes tortuous. Venous ulcers happen when there’s a break in the skin on the leg, usually around the ankle. 

DIAGNOSIS OF VARICOSE ULCER

  1. EXAMINATIONS
  • Inspection
  • Palpation – cough impulse test, multiple tourniquet test, Trendelenburg test
  • INVESTIGATIONS
  • Venography
  • Doppler ultrasound
  • Duplex ultrasound
  • Laboratory tests – to find out the underlying pathology

PROGNOSIS OF VARICOSE ULCER

  • In Ayurveda,the varicose ulcer is known as “sirajanya dushtavrana”

  • Excess consumption of ununctuous, cold food
  • Excessive physical exercise.
  • Suppression of natural urges.

Due to Vataprakopaka nidanas  (causative factors which increase vata)such as,  physical exertion,  straining,  for debilitated persons the vitiated vata enters the Siras  (veins)  causing the  Sampeedana  (Squeezing),  Sankocha (constriction),  and  Vishoshana  (the act of drying up)  which produces round and protruded Granthi (cyst) in the Siras (Veins), manifesting Siragranthi  (varicose vein). Vrana developed from siragrandhi is known as sirajanya dushta vrana.

  • Avoid food items that produce vata dosha imbalance like cold and dry food, hard to digest. Because vata imbalance aggravates the symptoms.
  • Consume easily digestible food items, cow’s milk
  • Avoid heavy exercises which further aggravate the conditions.
  • Take proper rest.

  • Nadi Shuddi Pranayam    – calms the mind and gets relief from aches and pains

The patient needs to be seated in a meditative posture with the head and spine erect, with the body relaxed. The patient has to close his one nostril (e.g. left nostril if using the right hand and vice versa) with the thumb and exhale completely through the other nostril. Again, he will have to breathe in deeply through the other nostril while the opposite nostril is still closed with the thumb.
